Patients must take appropriate doses of anticoagulation medicines in order to prevent clots from forming or bleeding. Since many medications, foods and diseases can affect your response to these medicines, frequent blood tests and discussions with your doctor or pharmacist are required.
The Anticoagulation Clinic at is run by pharmacists who are clinically trained to monitor and adjust anticoagulation medicines.
We offer blood testing, medication adjustments and education resources to increase the amount of time your blood tests are in the appropriate range.

What to Expect
Our clinic is staffed by pharmacists with advanced training in anticoagulation management. During your visit you can expect the following:
- Each visit takes 15 to 20 minutes
- The pharmacist will educate you on the use of anticoagulation medicines and answer any questions
- A small blood sample will be obtained by finger stick
- A pharmacist will make adjustments to your medication, if needed
- A follow-up appointment will be scheduled
- Anticoagulation management done by pharmacists reduces your risk for bleeding and forming blood clots
