Core Experiences

Ambulatory Oncology

The experience involves the provision of direct patient care in an outpatient/ambulatory care hematology/oncology setting.  This experience is designed to familiarize the resident with the management of patients with different cancer types and hematologic disease states along with supportive care that may be required.  The overall goal is to enable the resident to integrate their knowledge of pharmacotherapy, disease states, and pharmacokinetics in developing and accessing therapeutic plans and drug selection in the hematology/oncology setting.  Education of patients and staff, quality improvement activities and medication reconciliation will also be core components of the experience. 

Saint Joseph East Residency Program

Internal Medicine

The resident will acquire knowledge related to common disease states in hospitalized patients while rounding with an Internal Medicine physician.  Various topics that are covered include: asthma/COPD, renal disease, anemia, VTE prophylaxis, anticoagulation management, Parkinson's disease, pain management, and nutrition.  The resident provides complete clinical pharmacy services to each patient on the Internal Medicine service including the formulation and management of a pharmacotherapeutic plan.

Critical Care

The resident will develop skills including identification of therapeutic needs of the critically ill patient, reviewing medication profiles, monitoring for efficacy and toxicity, and providing medication recommendations and interventions in collaboration with the healthcare team. The resident attends daily ICU and Intensivist rounds.

Long-term Acute Care

The resident will provide clinical pharmacy services to patients that are admitted from outside hospitals for completion of antibiotic therapy, rehabilitation, or ventilator weaning.  Given the average length of stay of 25 days, the resident will have an abundance of opportunities for pharmacokinetic monitoring, anticoagulation consults, and overall pharmacotherapy management of complex pharmacotherapeutic regimens.

Infectious Diseases

Residents will participate in daily Antimicrobial Team service activities including prospective review of culture and susceptibility data for patients receiving broad-spectrum and/or restricted antimicrobial agents. Residents will also provide support and recommendations for the selection and streamlining of antimicrobial therapy. Additional activities include one-week rounding with the ID consult service (when availability permits), participation in ID topic discussions, and presentation of an ID journal club and lecture. Additionally, the resident will participate in the facility’s sepsis measures “code sepsis” review.

Emergency Medicine

The resident will provide clinical pharmacy services to patients in the Emergency Department. Area of emphasis include: toxicology, cardiology, infectious disease, neurology, medication reconciliation, various consultations, proactive medication error prevention, and code blues, stroke, sepsis coverage. The resident will also complete a quality improvement project to improve the care of patients at Saint Joseph East. The resident will attend daily rounds with the ED physician and be exposed to the different patient care issues unique to the ED.

Elective Experiences

Cardiology

The resident will become familiar with the management of common cardiac diseases.  Areas of emphasis include: acute myocardial infarction, heart failure, coronary artery disease, interventional cardiology, hypertension, anticoagulation, arrhythmias, pharmacokinetic and pharmacodynamic principles.  This rotation is designed to provide the resident with the tools to help integrate knowledge of cardiovascular guidelines and specific patient parameters into an individual evidence-based therapeutic plan while rounding with the Kentucky Cardiology group.

Pulmonary Medicine

The resident will attend daily in-patient rounds with a Pulmonologist at Saint Joseph East and participation in pulmonology clinic (as availability permits).  The resident will gain further understanding of basic pulmonary pathophysiology and identify the unique therapeutic needs of pulmonary patients.  The resident will gain insight into the management of common pulmonary diseases and become familiar with evidence-based guidelines for diagnosis and treatment of such diseases.

Neonatology

This experience allows the resident to gain experience in providing pharmaceutical services to the unique neonate patient population. The resident will attend daily rounds with the Neonatologist at The Women's Hospital at Saint Joseph East. Topic discussions will focus on the treatment of hyperbilirubinemia, fluid and electrolyte management, nutrition, neonatal growth and metabolism, and infants of mothers with diabetes.

Critical Care Selective

The resident will build on skills developed during the required critical care rotation. Critical Care is a prerequisite for the Critical Care Selective rotations. The resident attends daily ICU and Intensivist rounds and participates in more detailed critical care literature review.

Infectious Diseases Selective

Infectious Diseases is a pre-requisite for the Infectious Diseases Selective rotation. Residents will participate in daily Antimicrobial Team service activities and also provide support and recommendations for the selection and streamlining of antimicrobial therapy. Additional activities include three-week rounding with the ID consult service (when availability permits), participation in ID topic discussions, and presentation of an ID journal club and lecture.

Precision Medicine

The resident will main an in-depth understanding of pharmacogenomics and precision medicine as it relates to the care of cancer patients. Residents will increase their knowledge base in pharmacogenomic and precision medicine definitions, databases and guidelines. They will apply this information by critically evaluating next generation sequencing results and ultimately leading a multidisciplinary molecular tumor board review. They will participate in documentation and collaborate with other healthcare professionals such as research nurses, genetic counselors and oncologists.
