LEXINGTON, Ky. (Sept. 6, 2023) – The Saint Joseph Hospital Foundation has received $1,213,204 in funding from CommonSpirit Health’s Mission and Ministry Fund to continue to serve underserved patients in central and eastern Kentucky. The grant, part of the Community Health & Wellbeing program, will support the continuation of the foundation’s Total Health Roadmap Initiative, which integrates screenings for basic human needs into primary care settings.
With this funding, the Total Health Roadmap Initiative will continue to benefit Fayette, Madison and Laurel counties.
“CHI Saint Joseph Health is dedicated to spreading humankindness throughout our communities and serving underserved patients, but that doesn’t stop at physical health,” said Delaine Thiel, vice president, philanthropy, Saint Joseph Hospital Foundation. “Social and psychological care are integral to basic human needs, which is why we work diligently through the Total Health Roadmap Initiative to address all human needs, and we are thankful to continue expanding these important efforts.”
The initiative addresses “total health” by integrating a universal screening tool and community health workers into the systems of care. These screenings focus on social determinants of health, such as food insecurity, transportation and safe housing access, dental and eye care, quality child care, and access to behavioral health services and social support. CHI Saint Joseph Health connects these patients to vital community resources to address their total health needs and help improve their health outcomes.
