CHI Saint Joseph Health Guide to Obtaining Medical Records
To obtain medical records from CHI Saint Joseph Health, please download and mail the completed authorization form and a copy of a photo ID to the appropriate facility.
To obtain records from a CHI Saint Joseph Medical Group physician practice, please contact the practice directly to obtain the specific mailing address.
Medical Record Copy Fees
- For records sent directly to a provider or other health care facility for continuing care, there is no fee.
- For patients only, below are the standard fees to produce a copy of your medical records.
- Patients are entitled under Kentucky law to one copy of their medical record FREE of charge. See Kentucky Revised States Section 422.317. The following fees may charged by a health care provider for furnishing a second copy of the patient's medical record upon the patient’s request.
- Electronically maintained records delivered in electronic format $6.50
- Electronically maintained medical records combined with paper records delivered in Electronic format are billed at $6.50 (for the electronic portion) + $0.07 per page (for the paper portion) for labor involved in creating an electronic copy of the records maintained on paper
- Electronically maintained records delivered on paper are $0.90 for labor cost to create the paper copy of the record maintained electronically plus $0.05 per page for paper and toner
- Paper records delivered in electronic format are $0.07 per page for the labor involved in creating an electronic copy of the records
- Paper records delivered on paper are $0.12 per page
- Plus, postage and applicable taxes
Transferring Your Medical Records
Our Medical Records Department is also available to assist with the transfer of records to a new provider. Please ask your provider to contact our Medical Records Department by calling the appropriate facility below.
