Get a Second Opinion

Through our affiliation with Cleveland Clinic Cancer Center, a nationally ranked cancer program, you now have greater access to second opinions and breakthrough treatments right here in Lexington. Call our cancer care referral center in Lexington at 859.313.HOPE (4673) or 844-940-HOPE (4673) to speak with one of our referral coordinators. They are available to walk you through the referral process, assist with scheduling and help you get a second opinion for your cancer diagnosis and treatment.

New Patient Information Packet

After you schedule your first appointment at one of our Cancer Care locations you will be mailed a new patient packet. This packet will include new patient paperwork that you may fill out and bring with you to your appointment.

Patient Portal

For your convenience, MyChart patient portal provides online access to your oncology medical records. You can request appointments and prescription refills, view test results, and much more. It’s safe, secure and easy to get started. Ask your cancer care team how to sign-up today.

Medical Records Request

To gain access to your health information from CHI Saint Joseph Health, you’ll need to complete our Release of Information form and mail it to the appropriate facility, along with a copy of your picture ID. Download your form, which includes mailing addresses and complete instructions.
