We’ve made it easy to refer your patients for cancer care and are committed to getting them in quickly with our specialists and providing high-quality, compassionate care at every step. We also keep an open line of communication with you about their treatment plan and progress.
You or your staff may refer a patient by contacting the appropriate local office.
Please include any relevant records or information, such as:
- Lab and pathology reports
- X-ray and other imaging/scan reports
- Medical and physical history
- Treatment records: chemotherapy, radiation, etc.
- Operative reports
- Hospital discharge summaries
- Copy of your patient's insurance card, front and back
- Office notes
